Цель исследования - анатомо-функциональный анализ роли экстрасистолии как фактора риска развития атеросклероза артериальных сосудов. Обследовано 270 пациентов с суправентрикулярной и желудочковой экстрасистолией. Среди пациентов было 162 мужчин и 108 женщин в возрасте 63,4±6,6 лет. Пациентам выполнялась сфигмография, электрокардиография, эхокардиография и ультразвуковая допплерография магистральных артериальных сосудов. Определялись анатомо-функциональные и биомеханические показатели сердца и сосудов. Результаты исследования показали, что наблюдаются четыре структурно-функциональных эффекта экстрасистолии, играющие важную роль в качестве предикторов развития атеросклероза (растяжение артерий, механическое повреждение их эндотелия, появление дополнительных волн пульсации и формирование осложненной атеромы). Авторы считают, что фактором риска является не сама экстрасистола, а пульсовая волна первого пост-экстрасистолического сокращения, особенно после ранней экстрасистолы, возникшей до момента открытия митрального клапана, поэтому тактика лечения и целесообразность купирования экстрасистолии во многом должна определяться степенью нарушения гемодинамики при каждом конкретном варианте этого нарушения ритма.</p></abstract><trans-abstract xml:lang="en"><p>In recent years, researchers have mainly focused on the humoral and endogenous predictors of atherosclerosis, not paying due attention to anatomical, functional and biomechanical factors. The purpose of the study is an anatomical and functional analysis of the role of extrasystoles as a risk factor for atherosclerosis of the arterial vessels. A total of 270 patients with supraventricular and ventricular extrasystoles were examined. Among the patients there were 162 men and 108 women aged 63.4 ± 6.6 years. The patients underwent sphygmography, electrocardiography, echocardiography and Doppler ultrasound of the arterial main vessels. Anatomical and functional and biomechanical indices of the heart and vessels were determined. The results of the study showed that there are four structural and functional effects of extrasistoly, which play an important role as predictors of atherosclerosis (arterial sprain, mechanical damage to their endothelium, the appearance of additional pulsation waves and the formation of atheroma). The authors believe that the risk factor is not the extrasystole itself, but the pulse wave of the first post-extrasystolic contraction, especially after an early extrasystole, which occurred before the opening of the mitral valve, therefore the treatment tactics and the expediency of its at extrasystoles in many aspects is determined by the degree of hemodynamic disturbances in each specific variant of this rhythm disturbance.</p></trans-abstract><kwd-group xml:lang="ru"><kwd>экстрасистолия</kwd><kwd>анатомо-функциональный анализ экстрасистолии</kwd><kwd>атеросклероз</kwd></kwd-group><kwd-group xml:lang="en"><kwd>extrasystoles</kwd><kwd>anatomical and functional analysis of extrasystoles</kwd><kwd>atherosclerosis</kwd></kwd-group></article-meta></front><back><ref-list><title>References</title><ref id="cit1"><label>1</label><citation-alternatives><mixed-citation xml:lang="ru">Volodyukhin M.Yu.
