Механизмы инвазивной активности лимфоцитов в паренхиме щитовидной железы и ее морфологической перестройки при болезни Грейвса неизвестны. Цель исследования - изучение морфологических особенностей тироцитов фолликулов щитовидной железы и фенотипического состава интратиреоидных лимфоцитов при болезни Грейвса. В исследовании использованы ультразвуковые, гистологические, микро-морфометрические, иммунологические и иммуноферментный методы исследования. Материалом для исследования послужили операционные биоптаты щитовидной железы, полученные у пациентов при эпифасциальной тиреоидэктомии после достижения медикаментозного эутиреоза. В исследование было включено 58 женщин с болезнью Грейвса, средний возраст 40,4±13,2 лет, из них 18 (31,11%) с впервые верифицированным диагнозом и 40 (68,96%) с рецидивом заболевания. Результаты проведенных исследований показали, что исходом тканеспецифичного аутоиммунного воспаления и длительной тиреостатической терапии у пациентов с болезнью Грейвса является перестройка тиреоидного эпителия с гистологическими признаками хронического аутоиммунного тиреоидита, особенно выраженная при рецидиве заболевания. В группе пациентов с рецидивом болезни Грейвса повышено процентное содержание Т-лимфоцитов с фенотипом CD3+CD4+CD127LowCD25High и В1-клеток памяти (p&lt;0,001), при снижении содержания Т-активированных и цитотоксических активированных Т-клеток, а также популяции В1-лимфоцитов (p&lt;0,01), в сравнении с уровнем, установленным у пациентов с впервые выявленным заболеванием. Полученные данные позволяют предположить, что интратиреоидная Т-клеточная агрессия в фазу аутоиммунного гипертиреоза при болезни Грейвса, сменяющаяся медикаментозным эутиреозом и накоплением антитиреоидных препаратов в щитовидной железе проявляется в морфофункциональных реакциях паренхимы щитовидной железы и вносит существенный вклад в иммунопатогенез заболевания. Клинико-морфологические параллели фенотипов лимфоцитов крови и паренхимы щитовидной железы у пациентов с болезнью Грейвса доказывают наличие тканеспецифичного аутоиммунного воспаления и перестройки тиреоидного эпителия, с гистологическими признаками хронического аутоиммунного тиреоидита. </p></abstract><trans-abstract xml:lang="en"><p>The accumulation of T lymphocytes in the thyroid parenchyma in autoimmune hyperthyroidism is strongly restricted and may indicate the primary role of T cells in the development of Graves' disease (GD). However, the mechanisms of lymphocytes invasive activity in thyroid gland, as well as morphological rearrangement of the thyroid parenchyma in GD are still unknown. The aim of the present study was to establish the morphological features of thyroid follicles and the intrathyroidal lymphocyte subsets in GD. The results of the conducted studies showed that the outcome of tissue-specific autoimmunity and long term antithyroid therapy in GD patients is a rearrangement of the thyroid epithelium with histopathologic signs of  Hashimoto's thyroiditis, especially in case of  the disease reccurence. In group of GD disease relapse patients the percentage of CD3+CD4+CD127LowCD25High and B1 memory cells were increased (p&lt;0,001), with a decrease in the content of T-activated and cytotoxic activated T-cells, as well as the population of B1 lymphocytes (p&lt;0,01), in comparison with the level estimated in patients with the newly diagnosed disease.
