Цель исследования: получение новых данных по прижизненной анатомии и топографии предстательной железы в условиях ее доброкачественной гиперплазии по данным магнитно-резонансной томографии. Изучены и интерпретированы данные магнитно-резонансной томографии 195 урологических пациентов, из которых 100 пациентов с признаками доброкачественной гиперплазии предстательной железы и 95 пациентов без таковой. Оценивались линейные размеры в трех ортогональных плоскостях – сагиттальной, фронтальной и поперечной, объем предстательной железы, расстояния от нее до костных структур таза, площадь прилежания предстательной железы к мочевому пузырю и протяженность прилежания к прямой кишке. Выявлено, что частота пациентов с доброкачественной гиперплазией предстательной железы увеличивается с возрастом, в 1-м периоде зрелого возраста составляет 18,2%, во 2-м периоде — 46,7%, в пожилом — 81,8%, в старческом возрасте — 100%. Размеры и объем предстательной железы у таких пациентов также увеличиваются с возрастом: в 1-м периоде зрелого возраста средние значения объема простаты составляют 30,1 см3 против 23,1 см3 у остальных пациентов, во 2-м периоде – 41 см3 против 26,0 см3, в пожилом – 58,1 см3 против 23,3 см3 (соответственно), в старческом возрасте – 108,6 см3. Отмечается уменьшение расстояний от простаты до костных структур таза с возрастом, более близкое расположение ее основания к плоскости входа в малый таз. При доброкачественной гиперплазии предстательной железы также отмечается увеличение площади прилежания предстательной железы к мочевому пузырю и протяженности прилежания к прямой кишке. Представленные результаты могут быть использованы в диагностике доброкачественной гиперплазии предстательной железы у лиц мужского пола первой и второй возрастных групп.</p></abstract><trans-abstract xml:lang="en"><p>The study and analysis of intravital magnetic resonance imaging is relevant both from a theoretical and practical point of view as a supplement to the available data on the clinical anatomy of the male pelvis for radiologists and urologists. Purpose of the study: to obtain new data on intravital anatomy and topography of the prostate gland in conditions of its benign hyperplasia according to magnetic resonance imaging. The data of magnetic resonance imaging of 195 urological patients were studied and interpreted, including 100 patients with signs of benign prostatic hyperplasia and 95 patients without it. Linear dimensions were assessed in three orthogonal planes - sagittal, frontal and transverse, the volume of the prostate gland, the distance from it to the bone structures of the pelvis, the area of ​​​​adjacency of the prostate gland to the bladder and the extent of adherence to the rectum. It was revealed that the frequency of patients with benign prostatic hyperplasia increases with age, in the 1st period of adulthood it is 18,2%, in the 2nd mature age period – 46,7%, in the elderly – 81,8%, in old age - 100%. The size and volume of the prostate in such patients also increase with age: in the 1st period of adulthood, the average prostate volume is 30,1 cm3 versus 23,1 cm3 in other patients, in the 2nd period - 41 cm3 versus 26,0 cm3, in the elderly – 58,1 cm3 versus 23,3 cm3 (respectively), in old age – 108,6 cm3. There is a decrease in the distance from the prostate to the bone structures of the pelvis with age, a closer location of its base to the plane of the entrance to the small pelvis. With benign prostatic hyperplasia, there is also an increase in the area of ​​​​adjacency of the prostate gland to the bladder and the length of adherence to the rectum.
